Covid Vaccination Update
30th March 2021
All Llanyravon patients will be vaccinated at Cwmbran Mass Vaccination Centre.
If you are over 50 and have not been contacted for a Covid vaccination - please contact the MVC to arrange an appointment on 0300 303 1373
If you are 18-65 and have a long term health condition that usually requires you to have a flu vaccine and have not been contacted, please email us on email@example.com to review your notes. Only patients with SEVERE ASTHMA - resulting in a hospital admission and frequent courses of oral corticosteroids will be included in this cohort. All other asthmatic patients will be called soon.
Only 1 carer per vulnerable adult is entitled to a covid vaccine. The carer should provide over 35 hours care a week for the vulnerable adult. Please register yourself as a carer here! YOU DO NOT NEED TO CONTACT THE SURGERY - PLEASE FILL IN THIS FORM FOR PRIORITY VACCINATION
